Block Assignments

Posted below are the original block assignments released by the Group Director's Office and approved by the Curriculum Committee. The attending faculty member at the Children's Dental Center in Inglewood will be using this list as a basis for attendance. Attendance at this block assignment, like that of other block assignments, is mandatory. Clinic is scheduled Tuesday, Wednesday, and Thursday of your assigned week.

If you have an urgent schedule conflict, you must go through the official block change process. Obtain the official form, Request to Change Block Assignment. Fill it out and put in in the "Request" pocket of the folder posted near my office, 23-011. I will complete approvals by 5:00 PM on Tuesdays and Fridays. You may then pick up the approved notice and submit it to your Group Practice Administrator for entry into the SOE system. Approvals must be completed at least 2 weeks before your requested block change
